The UB-04 form is a medical insurance reporting form for facility billing replacing the UB-92. This form is required by many insurance carriers for drug and alcohol rehab facilities, eating disorder clinics, etc. It is much different than the CMS1500 and requires different information than the CMS1500 form such as Rev codes, type of bill, and occurrence codes.
